How can propulsion work "inside" a vacuum?

Present space travel can only work off of the "slingshot" effect, from one gravitational atmosphere to the next, i.e., jumping(propelling) from planet to planet. But how can other space travel and maneuvering be possible, including starting and stopping in a dead space vacuum?
